Trental during pregnancy: analogue and dropper, indications for use, reviews and which is better, what Curantil is for

During pregnancy, various circulatory disorders may occur, which in any case are primarily dangerous for the fetus. In the second trimester, the expectant mother may develop a complication such as gestosis (or late toxicosis), which is associated with stagnation of blood in the capillaries. The beginning of this process is manifested in the appearance of edema, then changes occur in the kidneys, which are characterized by the appearance of protein in the urine, and, finally, this all concerns the brain (the woman is bothered by headaches, convulsions, and in severe cases, coma is possible).

Due to the fact that in our time all expectant mothers are under the constant supervision of an obstetrician-gynecologist, cases of severe gestosis are extremely rare (they are usually treated immediately at the stage of edema appearing).

Another “known” reason that can provoke circulatory disorders during pregnancy is FPI (fetoplacental insufficiency). Usually, its development is provoked by spasms of the blood vessels of the placenta, as a result of which the fetus “starves” due to the insufficient amount of oxygen and nutrients received. This process can be accompanied by the formation of blood clots in the blood vessels of the placenta. Even with minor feto-placental insufficiency, a child may develop mild perinatal damage to the nervous system, and developmental delays may also be observed. In severe forms of FPN, severe brain damage is possible.

Therapeutic effect

Trental is a drug that improves microcirculation.
The active ingredient is pentoxifylline, it helps to normalize the rheological properties of the blood itself, prevents the process of platelet cell aggregation, and also significantly reduces its viscosity. Thanks to treatment with Trental, the normal shape of red blood cells is restored, which allows for better oxygen transport. Along with this, a vasodilator effect is recorded and vascular resistance decreases. According to research, Trental is effective in treating circulatory disorders in the brain. The drug is also successfully used for many diseases that are accompanied by deterioration of blood circulation in the vessels, including degenerative changes against the background of pathology of the vessels of the inner ear.

It is worth considering that during pregnancy, blood clotting increases, this is fraught with further deterioration of uteroplacental blood flow. In this case, the use of the medicine helps to thin the blood and improve its circulation in the placenta.

But you also need to pay attention to the fact that a slight increase in platelet levels (during the 3rd trimester) is considered quite normal. This is a manifestation of the body’s specific defense against possible bleeding during childbirth. The development of true fetoplacental insufficiency is observed quite rarely; such a diagnosis is made after Doppler ultrasound in cases of detection of delays in fetal development.

Compound

Trental tablets contain the active ingredient pentoxifylline 100 mg, as well as additional substances: silicon dioxide, starch, talc, lactose and magnesium stearate.
The shell consists of talc, titanium dioxide, sodium hydroxide, methacrylic acid copolymer, as well as macrogol 8000. The infusion solution concentrate in 1 ml contains the active substance pentoxifylline 20 mg, as well as additional substances: injection water and sodium chloride.

Extended tablets Trental 400 contain pentoxifylline in the amount of 400 mg, as well as additional substances: magnesium stearate, povidone, talc, hyaetellose. The shell contains: benzyl alcohol, talc, macrogol 6000, hypromellose, titanium dioxide.

Use of Trental during pregnancy

Taking into account the severity of the pathological process and the woman’s condition, outpatient or inpatient treatment can be prescribed. When carrying out therapeutic therapy at home, it is prescribed to take 1 tablet. twice or thrice a day. It is preferable to take the medicine simultaneously with food. The duration of treatment is usually 2-3 weeks, during which time the mother’s health improves significantly.

In case of serious circulatory disorders in the later stages, when there is pain in the lower abdomen, it is recommended to administer the drug to the pregnant woman intravenously (dropper or injections). Treatment is carried out in a hospital setting. Also, the doctor may additionally prescribe medications that are used only externally.

The drug dosage is usually 100-600 mg per day. Slow administration of the drug is indicated - no more than 100 mg in 1 hour. Some patients complain that it is scary to do IVs, so this procedure should be carried out under the supervision of medical staff.

In severe conditions, continuous administration of the drug throughout the day is indicated (a dropper is placed). The dosage is calculated taking into account body weight, but it is worth considering that it should not exceed 0.6 mg per 1 kg of weight.

Typically, the jet infusion procedure lasts approximately 5 minutes; it is recommended to carry it out twice a day, using 100 mg of medication. The duration of treatment is determined individually.

Interaction

According to the annotation, Trental enhances the effect of medications that affect blood clotting (thrombolytics, anticoagulants of direct and indirect effects), antibiotics (cefotetan, cefoperazone, cefamandole and other cephalosporins), valproic acid . Pentoxifylline enhances the effect of oral hypoglycemic agents, insulin, and antihypertensive drugs. Cimetidine can increase the level of the drug in the blood and increase the severity of side effects. With the simultaneous use of other xanthines, excessive nervous excitement of patients is observed.

Is Trental dangerous during pregnancy?

The instructions for the drug contain information that if blood flow A1 is impaired (the supply of nutrients to the placenta and uterus is slowed down during pregnancy), the medicine is not prescribed during the first trimester, since a teratogenic effect may occur, that is, intrauterine retardation of children's development may be observed.

Prescription in trimesters 2 and 3 is indicated, possibly, and is carried out with the aim of normalizing indicators that reflect blood flow in the placenta, thus, the baby’s body will receive vitamins and nutrients in full, the formation and growth of organs will correspond to the development calendar. The dosage of the drug is determined by the weeks of pregnancy. Starting from 34 weeks. the drug should be discontinued to reduce the likelihood of bleeding. After childbirth, the need to continue treatment is determined individually.

It is worth remembering that drugs are prescribed taking into account the possible benefits to the fetus. Due to the dilation of blood vessels in the uterus, there is an increase in the speed of blood flow and blood circulation in small capillaries. In addition, Trental prevents thrombus formation. The main advantage of the drug compared to other drugs with similar effects is its slight negative effect on the functioning of the cardiovascular system.

It is worth noting that the medicine is prescribed only by a doctor; Trental cannot be taken on its own.

Trental droppers: how safe are they?

Very often, Trental is prescribed during pregnancy in the form of tablets. However, in particularly serious cases, tablets are replaced with droppers. However, is it really safe?

Now the opinions of doctors are divided into two camps.
