    Lactinet Richter is a progestin contraceptive that is taken orally. The gestagen component is represented by desogestrel. Like other progestin hormonal pills, Lactinet Richter is recommended to be taken during breastfeeding; it is also indicated for women who cannot take estrogen-containing contraceptives. It is worth noting that the contraceptive effect of Lactinet is ensured by suppressing ovulatory function, along with this there is an increase in the viscosity of secretions, namely cervical mucus.

    While taking desogestrel-based birth control pills (the first two months), the frequency of ovulation is no more than 1%. After completion of contraceptive therapy, ovulatory function is restored over the next 7-30 days, on average, the onset of ovulation is recorded at 17 days.

    In previous studies of the drug based on desogestrel, the Pearl index was established, the indicator was 0.4. It can be compared with the Pearl index of estrogen-progestin contraceptives (COCs). The drug taken has a direct effect on the estradiol level, reducing its concentration to a level characteristic of the initial stage of the follicular period. These contraceptive pills do not have a significant effect on hemostasis and metabolism, namely the metabolism of carbohydrates and fats.

    Absorption

    After oral administration, the gestagen-containing tablet is quickly absorbed by the mucous membranes of the gastrointestinal tract and is biotransformed into the active metabolite of the substance etonogestrel. The highest level of etonogestrel is observed in the blood 1.8 hours after the use of the hormonal drug. The bioavailability of the drug does not exceed 70%.

    Distribution

    The association of etonogestrel with protein components of the blood is 95.5-99% (mainly with albumin, to a lesser extent it binds with globulins).

    Metabolism

    The gestagenic component of the drug, desogestrel, undergoes hydroxylation and dehydrogenation processes, as a result of which it is converted into etonogestrel, which is an active metabolite. During metabolic processes, the formation of both sulfate and glucuronide compounds is observed.

    Removal

    The half-life of metabolic products is 30 hours. Removal of free steroids and compounds from the body is carried out by the renal system and intestines.

    Lactinet during breastfeeding is excreted along with mother's milk. When consuming milk in a volume of 150 mg per 1 kg per day, a child can receive up to 0.05 mcg of gestagen per 1 kg of weight. Lactinet is absolutely safe for the baby during breastfeeding.

    Mechanism of action and indications

    The contraceptive effect of Lactinet is ensured by the following mechanisms:

    • inhibition of the ovulation process;
    • increasing the viscosity of cervical mucus, which makes it impermeable to sperm and prevents their penetration into the uterine cavity.

    Lactinet is prescribed to prevent unwanted pregnancy, especially during lactation or in case of contraindications to the use of combined hormonal contraceptives.

    How to take Lactinet

    The first pill from the package should be taken at 1 MC, and there is no need to resort to additional contraceptive measures. You can also start hormonal therapy with 2-5 MC, but in this case you should take care about using other contraceptives during the first seven days of taking the pills.

    Switching from COC

    You will need to start taking Lactinet tablets the next day after the last COC tablet taken, but you should not use protection against pregnancy; the contraceptive effect remains.

    Switching from single-component drugs that contain exclusively a progestogen component (mini-pills, injections, intrauterine devices or progestin-releasing implants)

    You can take the pills on any day of the MC; if you are injecting a contraceptive, you can take it on the same day when you need to take the next hormonal injection. If an implant was previously used - on the same day after the removal procedure.

    In the cases described above, it is worth resorting to a barrier method of protection for seven days of taking the pills.

    Features of use after an abortion performed in the first 3 months of pregnancy

    The pills are taken on the same day as the operation; there is no need to use contraception; pregnancy will not occur.

    Taking progestin pills after childbirth or after a late abortion (2nd trimester)

    Usually, taking the hormonal drug begins after 28 days. from the moment of artificial termination of pregnancy or after childbirth. If a gestagen-containing drug is taken later, there is a need to use a barrier method of contraception in order to avoid becoming pregnant.

    If you have previously had unprotected sexual contacts, you first need to exclude a possible pregnancy or wait until the start of MC after childbirth.

    Missing pills

    If a woman takes a hormonal drug 12 hours late, the contraceptive effect of the pills may decrease. It is not recommended to stop taking the drug; there is no need to use other contraceptive measures; let the woman take hormones as usual.

    If more than 36 hours have passed since taking the previous pill, the likelihood of pregnancy increases. Women taking gestagens should take the pills as soon as they remember the omission, and should use protection for the next 7 days. Next, you take contraceptive pills as usual.

    The possibility that a woman became pregnant on Lactinet if she missed a mini-pill in the 1st week of contraceptive therapy cannot be excluded if the previous 7 days. had unprotected sexual contact.

    Taking a mini-pill for vomiting and diarrhea

    If a disturbance in the functioning of the gastrointestinal tract is observed after 3-4 hours from the moment of taking the hormonal pill, there is a high probability of incomplete absorption of its components. The same measures should be taken as if you missed pills. It is best to take the pill from the new pack and continue taking the mini-pill from the old pack.

    Lactinet and menstruation

    Even if the mini-pill is taken correctly, menstrual dysfunction may occur. If you have breakthrough bleeding or heavy menstrual-like discharge, you should consider changing your contraceptive to another drug that will protect against pregnancy.

    If amenorrhea (no menstruation) occurs while taking the mini-pill, it is recommended to conduct a pregnancy test.

    How to stop drinking Lactinet

    Complete taking the pills from the package, the MC will be restored over the next 3 months.

    Use during pregnancy and pregnancy

    If you took the mini-pill and became pregnant, consult your doctor and stop taking the drug, be sure to inform your gynecologist and say: “I’m taking Lactinet.” After a woman has stopped taking hormonal medication, it is necessary to undergo an examination and rule out possible defects and disorders in the development of the embryo.

    Before using a gestagen-containing drug when breastfeeding after childbirth, you should consult with an obstetrician-gynecologist. Although hormonal components pass into breast milk, they do not in any way affect the development and growth of the baby. Therefore, breastfeeding is not a contraindication to taking the mini-pill.

    If a woman chooses these hormonal contraceptives after childbirth, she will need to visit a gynecologist every six months while using them.

    Contraindications

    Lactinet is contraindicated in the presence of a number of conditions.

    These include:

    • the presence of venous thromboembolism in the past or present;
    • severe liver disease at the moment or in the past (until normalization of liver function tests);
    • presence or suspicion of malignant hormone-dependent tumors (including breast cancer);
    • bleeding from the vagina of unknown etiology;
    • established or suspected pregnancy;
    • children and adolescents up to 18 years of age;
    • lactose intolerance, lactase deficiency;
    • hypersensitivity to desogestrel or other components of the drug;
    • long-term immobilization due to surgery or illness.

    Precautionary measures

    The mini-pill should be taken with caution when:

    • Development of resistant hypertension that arose while taking a hormonal contraceptive, or low effectiveness of antihypertensive treatment
    • Diagnosing systemic lupus erythematosus
    • Diabetes mellitus
    • Severe chloasma or a tendency to develop it
    • Herpetic rashes
    • Porphyria.

    It must be taken into account that one pill contains 67.4 mg of a substance such as lactose.

    special instructions

    If pregnancy occurs, the drug should be stopped immediately.

    It should be remembered that the use of oral contraceptives does not protect against HIV infection and other sexually transmitted diseases.

    Before starting treatment if you have any condition or risk factor, you must carefully weigh the expected benefits and possible side effects of the drug.

    Patients with diabetes mellitus should be under strict supervision during the first months of using Lactinet.

    Taking the drug helps lower the level of estradiol concentration in the blood to a value corresponding to the early follicular phase.

    In some cases, taking the drug may be accompanied by the occurrence of chloasma; the risk of its occurrence is especially high in women with a history of chloasma during pregnancy.

    During the course, patients prone to skin hyperpigmentation should avoid solar and ultraviolet irradiation.

    Lactinet, as a rule, inhibits ovulation, but do not forget about the possibility of developing an ectopic pregnancy. This must be kept in mind when making a differential diagnosis when a patient develops abdominal pain or amenorrhea.

    Since the drug contains lactose monohydrate (67.445 mg per tablet), women with lactose intolerance, or those with lactase deficiency or glucose-galactose malabsorption are not recommended to take the tablets.

    Cross-drug interactions

    When taking drugs that induce microsomal enzymes, the likelihood of breakthrough bleeding increases, against the background of which the contraceptive effect of the pills may decrease. Therefore, while taking a contraceptive, you should not drink drugs that are hydantoin derivatives.

    When taking rifampicin and for a month after treatment with this drug, you will need to protect yourself from pregnancy by other methods.

    The contraceptive effect of a gestagen-containing agent is reduced when taking certain antibacterial drugs and enterosorbents.

    The most effective mini-pills

    Pharmacy display cases are simply full of pastel-colored packages in which the treasured dragees are hidden. The most effective of them:

    • Lactinet;
    • Janine;
    • Charosetta;
    • Logest.

    The drug Lactinet is most often prescribed to all categories of women, especially nursing mothers. This statistics is explained by the fact that it is the Lactinet birth control pills that have a minimum of side effects and maximum effectiveness. Of course, even such a remedy is not without its small drawbacks, but they are not comparable to the high effectiveness of contraceptives.

    Side effects

    The most common side effect that develops while taking these mini-pills is disruption of the cyclicity of menstruation. Serious circulatory dysfunction, breakthrough bleeding or amenorrhea cannot be excluded.

    To control MC, you will need to keep a diary, which will help record the beginning and end of menstruation. According to the instructions, you will need to undergo regular medical monitoring when taking the mini-pill.

    While using a contraceptive, you may experience nausea, sudden emotional swings, chest pain, decreased libido, severe headaches, and weight gain.

    Rules for taking the mini-pill

    Despite all the variety of contraceptive drugs, their instructions for use are almost always the same. The tablets should be taken daily , preferably at the same time. You should not skip taking birth control pills. Taking the pills begins on the first day of menstruation and at the end of taking one package, the woman begins to have discharge. The girl should start taking the next batch of mini-pills after finishing the first blister, or after a seven-day break.

    This continuous supply of hormonal pills into the body continues exactly until the moment a woman decides to become a mother. Mini-pills are usually prescribed to women who have already given birth, and until this point you can use condoms, vaginal suppositories or special lubricants.
